Cleaning Your PECOS Profile: Why “Ghost Locations” Will Flag Your 2026 Bid

Your PECOS profile should be a mirror of your business. But for many providers, it’s a history book—full of old addresses, closed branches, and “storage units” listed as practice locations.

In 2026, these “Ghost Locations” are dangerous.

CMS is scrubbing the database before the Bid Window opens. If they find an address that doesn’t match reality, they don’t just delete the address—they flag your whole organization for fraud.

What are auditors looking for?

  • The Closed Satellite: You closed your office in County B last year but forgot to file the Form 855S to delete it.
  • The “Virtual” Office: You listed a Regus or UPS Store mailbox as a “practice location” to get a local license. (CMS hates this and will immediately revoke billing privileges).
  • The Warehouse: You listed a shipping warehouse as a “Retail” location, but it has no signage and no hours for the public.

CMS contractors (NSC) are conducting unannounced site visits.

  • The Test: They drive to the address listed in PECOS.
  • The Failure: If the door is locked during “posted hours,” or if the sign is missing, they mark it “Non-Operational.”
  • The Result: PTAN Deactivation within 10 days. You cannot bid with a deactivated PTAN.

Before you register for the bid, you must perform a “Search and Destroy” mission on your own data.

  1. Delete old locations via Form 855S.
  2. Verify hours of operation match the sign on the door.
  3. Confirm phone numbers are active and answered by professional staff (not personal voicemails).
